Site visits should also be requested through the form below.  Plant disease, insects, and weed identification assistance should include good quality images. Images should be submitted to doug_clement@ncsu.edu. Information in the “Assistance Needed’ section of the form should also include:

What percent of plants infected?
Are the symptoms widespread or just a single plant?
What are the plant symptoms or signs?
What are the pest signs (boring, mining, chewing etc…)
What has been the pesticides used?
What are the fertilizers used?
Site conditions?
Has there been any soil additives (mulch etc)?
