12/12/2018 – Below please find the link to the mandatory Annual Survey for the NC Industrial Hemp Program. Each year in December, Hemp License Holders receive this annual survey as part of the research component of their license. Failure to complete this survey could cause a license holder to lose their license or be ineligible for licensing in the future.

If you did not grow ANY hemp in 2018, you DO NOT have to complete the survey. But you MUST send an email to IndustrialHemp@ncsu.edu with your license number and state the following:

“My license number is ______. I declare that I did not grow any hemp in 2018. I am exempt from completing the 2018 grower survey.”

BEFORE YOU BEGIN:
1.) Gather your hemp records! Gather information related to your farm, growing conditions, planting and harvesting data, and business information. Have those records easily available before you begin the survey.

2.) Once you begin the electronic survey, you MUST complete it at that time. You cannot leave the survey and come back to it. The survey should take 20-30 minutes to complete, but please allow for up to 45 minutes, especially if you have a lot of information to enter. Please allow sufficient time to complete the survey completely and accurately.

3.) There is also an OPTION to add important documents that researchers may find helpful if you are willing to provide them: for example soil tests, THC tests, or CBD oil profiles, etc. Please scan those into files on your computer before you begin to have easy access to them. (not required)

Take the online survey (Preferred method)

Instructions for Paper Copy

The deadline to complete the survey is January 31, 2019.

The NC Industrial Hemp Pilot Program is first and foremost a research program. By growing hemp in the state of North Carolina, you become a citizen scientist.
There are only two types of hemp grower’s licenses available:
(1) Research Only
(2) Research with Intent to Market

Most people apply for and receive a “Research with Intent to Market” grower’s license. This license type allows the grower to sell his/her hemp crop into the marketplace, but it does not remove the research component to the license.

Each year, in December, license holders receive a survey with questions about their experiences growing hemp in the NC Industrial Hemp Pilot Program. In order to remain in compliance with the terms of your grower’s license, you must complete and return this survey by the due date.

On behalf of the Industrial Hemp Commission, North Carolina State University (NCSU), North Carolina Agricultural and Technical State University (NC A&T), and the Department of Agriculture and Consumer Services (NCDA&CS), thank you for being a part of the NC industrial hemp pilot program!

If you require a paper copy of the survey, please call or email the industrial hemp program.
